Key Responsibilities

As a Physical Therapist, your core responsibilities will include:

• Implementing individualized physical therapy interventions to prevent disability, relieve pain, and improve muscle functionality, all aligned with established treatment plans.
• Supervising a team of Physical Therapy Assistants to ensure compliance with the prescribed plans of care effectively.
• Conducting thorough diagnostic evaluations to assess patients' functional abilities and determining their rehabilitation needs.
• Evaluating the home environments of clients to recommend modifications that increase independence and safety.
• Providing home-based physical therapy sessions as warranted by the treatment protocols.
• Administering therapeutic exercises, manual techniques, and functional activities designed to enhance recovery.
• Employing various modalities, such as traction and massage therapy, to support patient rehabilitation.
• Evaluating and making recommendations for the adjustment of orthotic and prosthetic devices as part of comprehensive treatment.
• Contributing actively to the development, review, and updating of patient care plans.
• Maintaining accurate and timely clinical documentation to support the continuity of care and regulatory compliance.
• Participating in interdisciplinary team case conferences, contributing insights to treatment planning and evaluations.
• Guiding and educating clients and their families on effective usage of therapy devices and post-care instructions.
• Conducting functional assessments and community resource evaluations to support comprehensive care approaches.
• Carrying out additional tasks related to the organization's objectives as needed.

Patient Population / Client Focus

You will primarily work with diverse patient populations, including individuals recovering from orthopedic surgeries, sports injuries, chronic pain conditions, and neurological impairments. Your role is pivotal in helping clients achieve their recovery goals while promoting autonomy and functional independence.

Qualifications

The ideal candidate will possess the following qualifications:

• A Bachelor's Degree in Physical Therapy from an accredited program.
• A minimum of one (1) year of relevant experience in acute care or home healthcare settings.
• An active Physical Therapy licensure in the state of California.
• Current CPR certification to ensure patient safety.
• A valid driver's license and valid automobile insurance for travel between client locations.
• A demonstrated commitment to clinical excellence, integrity, and high-quality patient care.
• Exceptional interpersonal, communication, and collaborative skills necessary for effective teamwork.
• Familiarity with federal and state healthcare regulations to ensure compliance.
• The ability to advocate and effectively represent the organization's diverse service lines.

Preferred Experience

While not mandatory, candidates who possess experience in home health settings, familiarity with electronic health record systems, and previous involvement in interdisciplinary teams will be highly regarded. Compensation and Benefits

The salary range for this role is set between $100,000 and $120,000 annually, commensurate with experience and qualifications.
