Key Responsibilities\n

• Deliver therapeutic support and counseling services to individuals and families facing acute, long-term, or terminal mental health challenges, ensuring a comprehensive understanding of each patient’s needs.

• Maintain updated knowledge of government and private payer networks to facilitate appropriate care transitions for patients.

• Collaborate effectively with healthcare resources and community organizations to coordinate and optimize patient care plans.

• Serve as an advocate for patients and their families, ensuring access to necessary services while upholding patient rights and privacy.

• Conduct thorough assessments and screen for vulnerability or suspected abuse, with appropriate reporting as mandated by law.

• Engage in care system initiatives aimed at preventing patient readmissions, utilizing data-driven approaches to identify at-risk patients.

• Participate in transition conferences with patients and their families, developing clear and effective transition plans that prioritize the patient's needs.

Qualifications\n
