Key Responsibilities\n

• Provide individual and group psychotherapy sessions to patients coping with mental health conditions such as depression, anxiety, and grief related to terminal illness.

• Conduct comprehensive assessments to evaluate the emotional, behavioral, and social needs of patients and their families.
\n
• Develop personalized, evidence-based treatment plans that incorporate culturally competent practices tailored to each patient's unique circumstances.

• Collaborate closely with interdisciplinary teams, including nurses, physicians, and chaplains, to ensure cohesive care delivery and support for patients.

• Monitor and document patient progress using electronic health records (EHRs), making adjustments to treatment plans based on ongoing assessments and feedback.

• Facilitate educational sessions for patients and families, sharing vital information about mental health, coping strategies, and available community resources.

• Uphold ethical standards of practice by ensuring patient confidentiality and obtaining informed consent in all treatment interactions.

In this role, you will engage with a diverse patient population facing serious and terminal health conditions. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
