Key Responsibilities\n

• Conduct thorough assessments and evaluations of patients’ psychosocial needs to inform discharge planning and care management.

Patient Population / Client Focus\n

In this role, you will primarily serve patients facing terminal illnesses and their families. Understanding the sensitive nature of hospice care, you will provide empathetic support and guidance tailored to the unique emotional and practical needs of each patient. This position requires a deep understanding of the complexities of end-of-life issues and the ability to facilitate discussions about advance care planning, grief, and loss.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Current New York State licensure as a Clinical Social Worker (LCSW) is essential, with eligibility to provide services in a hospice setting.

Compensation and Benefits\n
