Work Environment

Key Responsibilities

• Deliver high-quality, patient-centered care by managing the full spectrum of therapeutic interventions for an assigned group of patients, while coordinating with physicians and the broader healthcare team.
• Perform comprehensive patient assessments, creating and implementing individualized care plans that prioritize goal achievement, patient engagement, and optimal rehabilitation outcomes.
• Provide skilled physical therapy treatments, including evaluations and therapeutic interventions that are fully compliant with legal and ethical standards of practice.
• Assess and evaluate the therapeutic, rehabilitative, and functional status of patients while collaborating with them to formulate their total plan of care.
• Administer therapeutic care directly in patients' homes, employing techniques designed to advance their recovery consistent with the established plan from their healthcare providers.
• Educate patients, families, and caregivers on rehabilitation exercises, therapies, and the effective use of therapeutic appliances and prosthetic devices, fostering independence and ongoing wellness.
• Manage caseload effectively by prioritizing patient needs, determining staffing requirements, and coordinating daily assignments and patient hand-offs.

Qualifications

• Current licensure as a Physical Therapist in the state of California is mandatory.
