This position is fully remote, allowing employees to balance their professional commitments with personal obligations from the comfort of their own homes. The role is integral to our Community Health Clinic, where you will collaborate with a diverse team of health professionals including psychologists, medical doctors, and community outreach coordinators to provide holistic support to clients and their families.

Key Responsibilities\n

Your primary responsibilities will include:

• Conducting comprehensive psychosocial assessments and developing individualized care plans to address patients’ emotional, social, and financial needs.

• Providing both individual and group counseling sessions that focus on issues such as grief, adjustment, trauma, and coping strategies.

• Facilitating transitions of care by coordinating with community organizations to ensure continuity of services and resources after discharge.
\n
• Offering emotional support and educational resources to patients and their families regarding treatment options available to them.
\n
• Conducting crisis intervention and providing immediate counseling support to patients in distress.
\n
• Maintaining accurate and timely documentation that complies with healthcare regulations and best practices.

• Participating actively in interdisciplinary team meetings to enhance patient outcomes and foster a collaborative work environment.

Patient Population / Client Focus\n

In this role, you will engage with a diverse patient population, including adults and children who are navigating various mental health conditions. This includes, but is not limited to, anxiety, depression, substance use disorders, and trauma-related issues. Your expertise will play a critical role in addressing the psychosocial factors that influence their health and encouraging a supportive environment for recovery.

Qualifications\n

The successful candidate will possess the following qualifications:

• A Master's Degree in Social Work from an accredited institution.

• An active state license as a Licensed Clinical Social Worker (LCSW).
