Key Responsibilities

• Conduct thorough patient assessments, including physical exams, medical histories, and necessary diagnostic tests.
• Utilize established standardized protocols to diagnose and manage health issues in accordance with best practices in nursing.
• Develop, implement, and evaluate individualized patient care plans aimed at promoting health and managing disease.
• Educate patients and their families about health maintenance, treatment plans, and lifestyle modifications necessary for improved health outcomes.
• Document patient interactions utilizing electronic health record (EHR) systems to maintain accurate, up-to-date medical information.
• Collaborate with healthcare peers to address complex patient needs and facilitate continuity of care.
• Participate in community outreach programs aimed at enhancing healthcare access and awareness.

Patient Population / Client Focus

You will primarily serve patients within the community setting, focusing on diverse populations who may face barriers to accessing traditional healthcare services. Qualifications

• Master's degree in Nursing from an accredited institution.
• Current certification as a Nurse Practitioner.
• Active, unrestricted nursing license in the state of New York.

• Dedication to advocating for vulnerable communities and addressing social determinants of health.

Preferred Experience

• A minimum of 3-5 years of clinical experience, particularly in community health, public health, or related settings.

Compensation and Benefits
