Key Responsibilities\n

• Conduct in-depth psychosocial assessments to evaluate patients' emotional, social, and financial needs, ensuring a holistic approach to care.

• Deliver individual and group therapy sessions focused on adjustment challenges, grief, and coping mechanisms, while tailoring interventions to meet the specific needs of each patient.

• Facilitate crisis intervention and provide emotional support during clinical emergencies, employing sound judgment and established protocols.

• Develop and oversee comprehensive discharge plans for patients, coordinating with community organizations to ensure that appropriate post-discharge services are in place.

• Engage with interdisciplinary teams to optimize treatment outcomes for patients, participating in case conferences and contributing social work expertise to treatment plans.

• Maintain accurate and timely documentation in accordance with hospital policies, treatment standards, and regulatory requirements.
