Work Environment

Work in a dynamic, client-focused environment as a Home Care Physical Therapist. You will be primarily responsible for visiting patients in their homes, collaborating with other healthcare providers, and delivering personalized care plans on-site. Key Responsibilities

• Conduct thorough evaluations of patients, reviewing their medical history, performing detailed assessments, and developing individualized treatment plans based on findings.

Patient Population / Client Focus

As a Home Care Physical Therapist, you will work with a diverse array of patients who may have various conditions, from orthopedic injuries to chronic illnesses. This role necessitates a strong understanding of rehabilitation principles and a compassionate approach to guide patients through their recovery journey in a familiar and supportive environment.

Qualifications

• Must possess a Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid state license for Physical Therapy practice in Virginia is required.
• Professional certification in Basic Life Support (BLS) from the American Heart Association is mandatory.
• While two years of prior experience is preferred, we are dedicated to supporting new graduates who show a passion for patient care.

Preferred Experience

• Background in home health care or outpatient rehabilitation settings is advantageous but not obligatory.
• Experience with patient evaluation and customized treatment plan design, particularly for individuals recovering from surgery or injury.
• Skills in using electronic medical record (EMR) systems for documentation and patient management.

Compensation and Benefits
