Key Responsibilities\n

• Conduct thorough and ongoing assessments tailored to the individual needs of clients, promoting effective treatment planning.

• Actively communicate with supervisors, peers, and family members involved in each client’s care to ensure collaborative support.

• Create and maintain a tentative schedule of client appointments, home visits, and observation sessions to enhance care continuity.

• Coordinate comprehensive home visits, including chart reviews and staff interviews, to gather pertinent data for treatment evaluations.

• Oversee the implementation of personalized program plans to guarantee optimal engagement in therapeutic processes.
\n
• Participate in interdisciplinary team meetings, contributing insights based on individual client progress and needs.

• Complete detailed monthly and quarterly progress notes, documenting client outcomes and treatment effectiveness.

• Organize and lead training sessions to ensure all staff are proficient in program implementation and documentation processes.

• Provide follow-up consultations and home visits to monitor client progress and adapt treatment approaches as needed.

• Maintain meticulous records, ensuring information is accessible to authorized parties while upholding confidentiality protocols.

• Serve as a liaison to the community and mental health networks, supporting development goals and advocating for client needs.

• Assist in creating and refining training materials aimed at enhancing program efficacy and staff competencies.

Patient Population / Client Focus\n

Qualifications\n
