Key Responsibilities\n

• Conduct comprehensive assessments for clients to identify their mental health needs accurately, tailoring therapeutic approaches accordingly.

• Maintain effective communication with supervisors, healthcare staff, and families, providing updates on client progress to ensure cohesive care delivery.

• Devise and share a tentative schedule for appointments, home visits, and observations to monitor client progress and service provision.
\n
• Organize and coordinate in-home visits that adhere to observation protocols while reviewing relevant charts and conducting staff interviews.

• Implement and oversee the effective execution of individualized program plans while evaluating their impact on client outcomes.
\n
• Engage actively in Interdisciplinary Team Meetings, bringing insights from your experiences to enhance collaborative treatment planning.

• Document monthly and quarterly progress notes meticulously to track client development and inform treatment adjustments.
\n
• Lead training sessions to equip team members with the necessary skills for effective implementation of program plans and documentation processes.

• Facilitate ongoing follow-up through interviews, home visits, and observations, ensuring sustained support for client well-being.

• Maintain accurate, organized records that are accessible to authorized parties and enhance overall treatment transparency.

• Serve as a liaison between our clinic, the public, and the broader mental health community, advocating for development goals and client interests.

In this role, you will work with a diverse population of clients, ranging from children and adolescents to adults, each presenting unique mental health challenges. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• An active and unrestricted professional license as a Counselor (LPC) in the state of Pennsylvania.
