Work Environment

Key Responsibilities

• Conduct thorough evaluations of patients' physical abilities and limitations in their home environments.
• Develop and implement personalized treatment plans based on your assessment and the specific goals of each patient.
• Utilize various therapeutic techniques, including exercise programs, manual therapy, and education on managing their condition effectively.
• Maintain detailed documentation of patient progress, updating treatment plans as necessary to reflect patients' evolving needs.
• Educate patients and their families on exercises, proper use of assistive devices, and strategies for enhancing mobility and function.
• Collaborate with multidisciplinary teams, including caregivers and other healthcare professionals, to ensure comprehensive patient care.
• Stay current with best practices and develop professionally by attending workshops, conferences, and engaging with professional organizations.

Patient Population / Client Focus

You will work with a varied patient population, including individuals recovering from orthopedic procedures, neurological conditions, and those with chronic pain. The role allows you to get to know your patients in their own environment, fostering a supportive relationship that is essential for effective rehabilitation.

Qualifications

• Doctorate in Physical Therapy (DPT) is preferred.
• Current state licensure or eligibility to obtain state licensure in Physical Therapy.
• Strong verbal and written communication skills to effectively engage with patients, families, and team members.
• Proficient in digital documentation and health care systems.
• Flexibility and adaptability in a dynamic home care environment.
• A commitment to ongoing learning and professional development.

Preferred Experience

• Minimum of two years of experience in a clinical setting is preferred, although recent graduates are encouraged to apply.
• Experience in home care or community-based therapy settings is advantageous.

Compensation and Benefits
