Work Environment

This position is based onsite, providing care directly in patients' homes throughout West McLean and surrounding areas. Working as part of a multidisciplinary team, you will engage with clients and their families in diverse settings, requiring strong communication and interpersonal skills to foster trusting relationships. Your clinical practice will be supported by a technology-driven environment that ensures seamless interactions with your colleagues and effective management of patient information.

Key Responsibilities

• Develop and implement individualized therapy plans to prevent disabilities, relieve pain, and enhance muscle function in accordance with each client's specific needs.
• Supervise Physical Therapy Assistants to ensure the effective execution of therapy plans and the highest quality of patient care.
• Conduct comprehensive assessments of clients' physical capabilities using diagnostic tools to evaluate muscle, nerve, and joint function.
• Assess clients' home environments to identify challenges to rehabilitation and recommend modifications to promote safety and independence.
• Administer a range of therapeutic modalities including manual therapy, therapeutic exercises, traction, and other physical agents as indicated.
• Evaluate and fit prosthetic and orthotic devices, providing recommendations for modifications as necessary.
• Prepare and maintain accurate clinical documentation including OASIS assessments, progress notes, and discharge summaries to monitor treatment and outcomes.
• Engage in care plan development and coordination with an interdisciplinary team ensuring holistic and informed treatment approaches.
• Provide education and support to clients and families on managing physical therapy programs and the use of therapeutic devices.
• Complete all required clinical documentation and administrative tasks promptly to align with organizational policies and maintain compliance.
• Participate in team meetings to review and update patient care strategies based on evolving needs and conditions.

Patient Population / Client Focus

The Home Care Physical Therapist will work primarily with patients across various age groups who are navigating recovery from surgeries, managing chronic conditions, or require rehabilitation after injury. These clients benefit significantly from therapy delivered in a familiar environment, allowing for tailored interventions that support their unique challenges in everyday settings.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• A valid physical therapist license (DPT) in the state of Virginia.
• Current CPR certification and a valid driver's license with proof of automobile insurance.
• Strong interpersonal communication skills and the ability to engage effectively with patients and families are essential.
• A passion for clinical excellence and a commitment to ongoing professional development.

Preferred Experience

• At least two years of experience as a physical therapist, ideally in homecare or acute settings.
• Proficiency in current best practices and a deep understanding of federal and state regulations governing home healthcare services.
• Experience in rehabilitating patients with various diagnoses, focusing on enhancing their quality of life.

Compensation and Benefits

This position offers a competitive salary ranging from USD 100,000 to 120,000 annually, in addition to a comprehensive benefits package.
