Work Environment

As part of our health care team, you will work onsite, primarily in patients' homes, with the chance to collaborate with various health professionals to optimize patient outcomes. Key Responsibilities

• Conduct comprehensive evaluations by reviewing patient history, performing relevant assessments, and determining effective treatment recommendations based on findings.

• Communicate evaluation results and develop treatment plans in collaboration with the patient and their family, ensuring their involvement in goal setting.

• Implement individualized treatment protocols that take into account the patient's specific needs, progress, and any prevalent changes in their health condition.

• Coordinate with other healthcare professionals to provide holistic care and recognize any changes in the patient's condition that necessitate adjustments in their treatment plan.

• Engage in discharge planning and follow-up care, meticulously documenting all aspects of patient care, including initial evaluations, progress updates, and discharge summaries.

• Exhibit effective time management and productivity, promptly attending meetings and upholding a well-maintained schedule.

• Contribute to department goals, staying compliant with regulatory standards and accreditation requirements.

• Proactively seek out continuing education opportunities to enhance professional skills and knowledge benefitting both personal growth and organizational competencies.

• Deliver exceptional customer service by being adaptable and responsive to patient needs, ensuring optimal access to care.

Patient Population / Client Focus

The Physical Therapist will be responsible for treating a diverse patient population, including individuals recovering from surgery, managing chronic conditions, or needing rehabilitation support for various musculoskeletal and neurological conditions. By working in a home care setting, you will engage closely with patients, enhancing their comfort and confidence while providing necessary therapeutic support.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required.

• Current licensure to practice as a Physical Therapist as required in Virginia.

• American Heart Association's BLS for Healthcare Providers CPR certification is mandatory.

• Exceptional communication and interpersonal skills, with a strong focus on patient welfare and professional collaboration.

Preferred Experience

Compensation and Benefits
