Key Responsibilities

As a Home Care Physical Therapist, your responsibilities will include:

• Directing individualized physical therapy treatment plans aimed at preventing disability, alleviating pain, and enhancing muscle function.
• Overseeing physical therapy assistants to ensure the effective implementation of treatment plans.
• Conducting thorough evaluations using diagnostic assessments to establish patients' functional abilities and limitations.
• Assessing patients' home environments to make recommendations that promote independence and rehabilitation.
• Delivering therapeutic interventions as prescribed, including manual therapy, exercise programs, traction, and application of physical agents.
• Fitting and adjusting prosthetic and orthotic devices, consulting with physicians for necessary modifications.
• Documenting patient progress through clinical records, including initial evaluations, care plans, and discharge summaries, ensuring compliance with OASIS standards.
• Collaborating with an interdisciplinary team during case conferences to develop cohesive treatment strategies.
• Providing education to clients and families regarding physical therapy devices and overall treatment programs.
• Staying available for consultation and supervision of physical therapy assistants during their provision of care.
• Completing additional duties as assigned to meet organizational objectives.

Patient Population / Client Focus

Your role will predominantly involve working with patients in their homes, many of whom may be coping with physical disabilities, recovery from surgery, or chronic conditions requiring ongoing therapy. Your expertise is vital in creating an environment where patients can thrive physically, emotionally, and socially.

Qualifications

The ideal candidate will possess:

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• An active PT license in the state of Virginia, free from any sanctions or disciplinary actions.
• A strong foundation in manual therapy techniques and an ability to design and implement effective exercise regimens.
• Proficient skills in using Electronic Medical Record (EMR) systems, particularly Web PT, for accurate documentation.
• Up-to-date knowledge of evidence-based physical therapy practices.

Preferred Experience

We value candidates who have prior experience in home healthcare settings, giving them an advantage in understanding the unique challenges that patients in their homes may face. Familiarity with OASIS documentation and collaboration with interdisciplinary teams will be beneficial in this role.

Compensation and Benefits

This full-time position offers a competitive salary range of USD 100,000 to 120,000 annually, commensurate with experience, along with a comprehensive benefits package that includes:
