This role is specifically designed for home care settings, meaning that your workplace is the patients' homes. This setting allows for a unique, personalized form of patient care, enabling therapists to directly observe and address challenges that individuals face in their daily environments. With a schedule of 40 hours per week and variable weekends, you'll enjoy the flexibility of conducting patient visits with supportive colleagues while utilizing resources from our central office.

Conduct thorough evaluations of patients with various functional limitations and disabilities to establish a comprehensive treatment plan tailored to individual needs.

Provide expert consultation to other healthcare professionals regarding a patient's ongoing care, ensuring a holistic approach to health management.

Develop and maintain effective communication with patients, their families, and interdisciplinary team members while adhering to the highest standards of customer service.

Document all aspects of patient care, including evaluations, treatment plans, and billing, in compliance with regulatory standards and internal protocols.

Engage in leadership responsibilities such as community outreach, continual quality improvement initiatives, and mentorship of support staff, aiding in the professional development of the team.

You will work predominantly with a diverse patient population comprised of individuals recovering from surgeries, managing chronic conditions, or rehabilitating from injuries. A Bachelor's Degree in Physical Therapy is essential for this role, with a commitment to maintaining professional competence.

Basic Life Support (BLS) certification is mandatory to ensure patient safety during any medical interventions.
