In this full-time role, you will be integral in guiding clients through their mental health journeys from the comfort of their homes, primarily serving individuals within a hospice setting.

Work Environment\n

This role is entirely remote, allowing for flexibility in managing your work schedule while prioritizing client needs. You will collaborate with a multidisciplinary team of mental health professionals, including psychiatrists and psychologists, ensuring a cohesive approach to client care. We also encourage a healthy work-life balance, supporting our clinicians with flexible scheduling, including some evening and weekend hours, to accommodate client appointments.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of clients to understand their unique mental health concerns, strengths, and available resources.

• Deliver individual, family, and group therapy sessions utilizing evidence-based treatment modalities, such as Cognitive Behavioral Therapy (CBT), Solution-Focused Therapy, and Trauma-Informed Care.

• Develop, implement, and routinely update individualized treatment plans that reflect client goals and therapeutic interventions tailored to their needs.
\n
• Engage in collaborative care efforts with other mental health professionals to ensure a well-rounded approach to each client’s wellness.
\n
• Provide crisis intervention, conduct risk assessments, and develop safety plans to offer support during critical times.
\n
• Facilitate engaging group therapy sessions, psychoeducation classes, and support groups, encouraging peer connection and community healing.
\n
• Maintain thorough and timely documentation of client progress and treatment in compliance with ethical and legal standards.

The ideal candidate will work primarily with individuals facing end-of-life issues or significant life transitions within a hospice care setting. Qualifications\n

• Master’s degree in Social Work from an accredited program is required.

Compensation and Benefits\n
