Key Responsibilities\n

• Conduct comprehensive bio-psychosocial evaluations to identify barriers to treatment and enhance patients’ overall health status.

• Develop and implement individualized treatment plans tailored to meet the specific needs of each patient, utilizing both individual and group therapy modalities.

• Collaborate closely with a multidisciplinary team, providing critical insights regarding patients’ psychosocial functioning, to foster coordinated and comprehensive care.

• Maintain accurate and timely documentation of social work activities in patients’ medical records, adhering to departmental and regulatory standards.

• Lead educational sessions for patients and their families on medical and therapeutic recommendations, promoting understanding and adherence to treatment plans.

• Assess advocacy needs for patients and families, connecting them with community resources that promote their well-being and self-determination.
