Key Responsibilities\n

• Conduct thorough assessments and evaluations for clients to determine their individual needs and treatment goals.

• Maintain ongoing communication with supervisors, colleagues, family members, and relevant individuals to ensure coordinated care and to monitor client progress.

• Prepare and provide a structured schedule for client appointments, home visits, and observational tasks to facilitate comprehensive care.

• Organize and execute home visits to conduct in-home assessments, interviews, and data analysis to enrich your understanding of clients’ situations.

• Ensure that treatment plans are effectively implemented and adjusted as necessary based on client progress and feedback.
\n
• Participate in Interdisciplinary Team Meetings, sharing insights and collaborating with other healthcare providers to enhance treatment strategies.

• Document client progress accurately through monthly and quarterly reports, maintaining our commitment to transparency and accountability.

• Offer training sessions to staff and team members, ensuring they are equipped to implement program plans and complete required documentation competently.

• Engage in follow-up interviews and observations to provide ongoing support to clients in need.

• Act as a liaison to the broader mental health community, fostering partnerships that promote strategic development goals.

• Assist in developing training materials and procedural documentation to continuously improve our program offerings.

Patient Population / Client Focus\n

You will work with a diverse range of clients, including adults and youth experiencing various mental health challenges. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• An active and unrestricted license as a Professional Clinical Counselor (LPCC) in California.

• A minimum of two years of counseling experience, preferably in a clinical environment.

• Demonstrated expertise in areas such as family therapy, couples therapy, substance misuse treatment, or trauma-informed care is highly preferred.
