Key Responsibilities

• Conduct comprehensive patient evaluations to assess physical abilities and limitations, utilizing evidence-based practices to design personalized treatment plans.
• Deliver individualized therapy sessions for an average of 7 patients daily, focusing on therapeutic exercises, manual therapy techniques, and patient education.
• Monitor patient progress closely, making necessary adjustments to treatment plans and documenting findings in our Electronic Health Record (EHR) system.
• Engage with patients and their families effectively, providing clear communication about treatment goals and progress, empowering them to take an active role in their rehabilitation.
• Collaborate with other rehabilitation professionals to ensure a cohesive treatment approach, participating in regular team meetings to discuss patient care strategies.

Patient Population / Client Focus

As part of our skilled nursing facility, you will primarily work with elderly patients and those recovering from surgeries, strokes, and other debilitating conditions. Understanding the unique challenges faced by this population is essential, and your role will be instrumental in helping them regain independence and improve their quality of life.

Qualifications

• A valid New York State Physical Therapist License is required.
• A Bachelor's Degree in Physical Therapy or a related field is necessary.
• At least two years of hands-on experience in a physical therapy setting, preferably within a skilled nursing or rehabilitation facility.

• Experience with patient populations in post-operative recovery and long-term rehabilitation.

Compensation and Benefits
