Behavioral Health Counselor - LISW or LPC Duties and Responsibilities: Provides counseling and support to individuals and families experiencing behavioral, emotional, mental health, substance use, and psychosocial challenges. Develops and implements individualized treatment plans using current behavioral health assessment, diagnostic, counseling, and treatment techniques. Counsels clients individually and/or in group sessions (as appropriate) to assist the client in overcoming individual or family issues, mental illness, or substance abuse. Monitors patient progress, evaluates treatment effectiveness, and modifies treatment plans as clinically appropriate. Counsels family members (as appropriate) to assist with treatment issues and to serve as a support system to the client. Refers clients to other supportive services as needed. Prepares and maintains reports and case histories. Assists in providing quality assurance and process improvement reports for improving quality of patient care and provides support to the Case Management Team, as needed. Maintains timely, accurate, and complete documentation in the electronic health record (EHR) in accordance with organizational policies and regulatory requirements. Performs other duties as assigned by supervisor. Qualifications: Education/Experience: Master's degree in Social Work or Counseling required. Current, unrestricted South Carolina licensure as a Licensed Independent Social Worker-Clinical Practice (LISW-CP) or Licensed Professional Counselor (LPC) required. One to three years' experience in a primary healthcare setting preferred. Must maintain a South Carolina Driver's license CPR certification required. #J-18808-Ljbffr Tandemhealthsc
