This full-time position offers the excitement of working in a home care environment, aiming to deliver exceptional therapy services and improve patient outcomes. Work Environment

This position is office-based at patients' homes within the Clifton, Virginia area. As a Physical Therapist, you will work closely with patients in their own environments, fostering independence and enhancing their quality of life. The schedule is Monday to Friday, from 9 am to 5 pm, providing a balanced work-life environment.

Key Responsibilities

In your role as a Physical Therapist, you will:

• Direct physical therapy treatment to prevent disability, relieve pain, and enhance muscle function, adhering closely to individualized care plans.
• Supervise Physical Therapy Assistants (PTAs) ensuring the effective implementation of treatment plans in a home setting.
• Conduct thorough evaluations of patients' functional abilities through a variety of diagnostic tests and assessments.
• Assess home environments to recommend adjustments which facilitate greater autonomy and rehabilitation for clients.
• Deliver prescribed physical therapy treatments, including manual therapy, therapeutic exercises, and treatments utilizing physical agents like traction and massage.
• Evaluate, fit, and adjust prosthetic and orthotic devices, advocating for necessary modifications to the overseeing physician.
• Contribute to the interdisciplinary team by actively participating in case conferences, care plan development, and evaluating the overall treatment strategy.
• Maintain meticulous clinical records, documenting all evaluations, care plans, progress notes, and discharge summaries efficiently.
• Engage with clients and their families to provide education on therapy techniques and device care, ensuring they are empowered to participate in their recovery.
• Fulfill additional assignments that support the company's mission and facilitate high-quality care delivery.

Patient Population / Client Focus

You will primarily work with a diverse range of patients, including those recovering from surgeries, managing chronic conditions, or needing rehabilitation after injuries. Our patient-centered approach means every treatment is tailored to fit the unique needs and goals of each individual, promoting not only physical well-being but also emotional and psychological recovery.

Qualifications

We are looking for candidates who meet the following qualifications:

• Graduated from an accredited physical therapy program.
• Possess a current physical therapy license in good standing for Virginia.
• Hold a valid CPR certification.
• Have a valid driver's license and auto insurance, allowing for the travel necessary in this role.
• Exhibit strong decision-making abilities and be capable of creative problem-solving in challenging situations.
• Demonstrate a commitment to clinical excellence and high standards of patient care.

Preferred Experience

While a minimum of one year of experience in either acute care or home care settings is required, candidates with experience in a rehabilitation-focused role are strongly preferred. Familiarity with OASIS documentation standards and interdisciplinary teamwork will enhance your candidacy.
