Case Manager — Homestretch

Homestretch is looking for a compassionate, organized, and driven Case Manager to join our team. In this role, you'll be the primary point of contact and advocate for clients as they work toward their goals — providing support, connecting them to vital resources, and helping them build a path forward.

What You'll Do

• Serve as the main point of contact between Homestretch and clients on your 8-10 client caseload, guiding and motivating them to set and achieve personal goals
• Conduct client assessments and evaluate program suitability alongside the direct services team.
• Coordinate client move-ins — from property inspections to lease signing and community orientation
• Advocate for clients with community and government agencies (mental health, medical, financial services, and more)
• Maintain regular contact through home visits and phone calls; build individualized service plans and track progress
• Connect clients to Homestretch programs like employment and credit counseling, ESL classes, and Life Skills workshops
• Manage documentation using Homestretch's confidential client database and HMIS system
• Partner with landlords to ensure well-maintained housing and positive relationships
• Support special programs and community events (camp, holiday shopping, back-to-school drives, and more)
• Participate in staff meetings, community events, and occasional evening/weekend programming (including monthly Life Skills sessions)

What You Bring

• Bachelor's degree in Social Work, Psychology, Human Services, or a related field (MSW a plus)
• At least 2 years of experience in human services
• A strengths-based, trauma-informed approach to client care
• Strong organizational skills, sound judgment, and a non-judgmental, professional demeanor
• Comfort with Microsoft Word and Excel
• The ability to stay composed under pressure while protecting client confidentiality
• Excellent English communication skills, both written and verbal
• A valid driver's license, auto insurance, and reliable access to a vehicle
• Bonus: fluency in Spanish, Arabic, or Farsi
