Key Responsibilities\n

• Conduct comprehensive patient intakes that focus on diagnostic assessments, functional evaluations, and the formulation of treatment plans.

• Provide brief, targeted interventions for patients experiencing mental health challenges, following evidence-based therapeutic approaches.

• Advise primary care professionals on managing patients' mental health needs while ensuring their integration into holistic care plans.

• Develop and implement follow-up strategies to monitor patient progress and make necessary adjustments to treatment plans.

• Create relapse prevention strategies aimed at maintaining patient stability and preventing exacerbation of mental health issues.

• Coordinate behavioral health services, ensuring seamless communication between primary and specialty care providers as needed.

• Document patient progress comprehensively in accordance with agency guidelines, maintaining confidentiality and accuracy.

• Provide educational resources and self-management strategies to empower patients to actively participate in their recovery.

• Serve as a consultation resource for clinic management on behavioral health matters.

• Adhere to the ethical standards required of clinical professionals and carry out additional tasks as assigned.

Qualifications\n

• A Master’s Degree in Clinical Counseling from an accredited institution.

• Current and valid LPC (Licensed Professional Counselor) license.
