Key Responsibilities\n

As a Licensed Professional Counselor, the successful candidate will be responsible for a variety of essential duties, including but not limited to:

• Conducting comprehensive assessments and evaluations of clients to identify needs and inform treatment planning.

• Regularly communicating with supervisors, interdisciplinary team members, family members, and other stakeholders to report on clients’ progress.
\n
• Creating and providing a preliminary schedule that details appointments, necessary home visits, and observations to support ongoing treatment.
\n
• Coordinating in-home visits as required, which may encompass observations, chart reviews, and data collection to ensure that client care is comprehensive and effective.
\n
• Implementing individual treatment plans and monitoring their effectiveness through consistent follow-up and adjustments as warranted.
\n
• Participating actively in team meetings to collaborate on case reviews and strategize on treatment methods.
\n
• Documenting monthly and quarterly progress notes to maintain accurate records of client progress and treatment efficacy.
\n
• Organizing and facilitating training sessions to ensure that all staff members are competent in the implementation of treatment plans and are well-prepared for their roles.

• Acting as a liaison with community resources and stakeholders to enhance client support and reach developmental goals.

• Engaging in regular review of client records to uphold the standards of care and guarantee information accessibility for authorized personnel.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related discipline is required.

• Must possess active and unrestricted licensure as a Professional Counselor in the state of Arizona.

• Specialization in family therapy, couples therapy, substance abuse counseling, or trauma-informed care is highly desirable.

• A solid understanding of legal and ethical standards governing mental health practice.
