Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of children to determine their developmental needs and strengths.

• Participate in the development and implementation of Individualized Family Service Plans (IFSPs) to support children and their families.

• Monitor and assess outcomes as a collaborative member of the care team, ensuring access to necessary resources and services.

• Engage with families to provide essential support and information related to enhancing their children's development and well-being.
\n
• Adhere to all Federal and State regulations governing service delivery in the community health sector.

• Utilize evidence-based practices and Natural Learning Environment Practices to develop personalized treatment strategies.

• Provide coaching and guidance in natural settings to promote learning and development aligned with each child's unique treatment goals.
\n
• Ensure compliance with health and safety standards to protect the well-being of staff and clients.

Patient Population / Client Focus\n

The focus of this role is primarily on children aged birth to three years old who demonstrate disabilities or developmental delays. Our services aim to facilitate early intervention and harness each child's potential through tailored support strategies. Working with families is crucial, as our approach prioritizes collaboration and empowerment, ensuring that families feel equipped to support their child’s development.

Qualifications\n

• Master's Degree or Doctorate in Social Work from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) with the Department of Public Health.

• A valid driver's license is required for home visits as needed.

Compensation and Benefits\n
