Key Responsibilities\n

• Conduct thorough assessments of clients’ mental health needs and develop personalized treatment plans that may include pre-crisis and crisis intervention strategies.

• Utilize evidence-based screening tools as dictated by our clinical team to ensure accurate diagnostics and effective treatment paths.

• Maintain meticulous documentation and communications regarding client progress, adhering to all regulatory standards and organizational policies.

• Engage in collaborative efforts with multidisciplinary teams to create and implement positive behavior support plans tailored to seniors.

• Act as a liaison with external service providers, state agencies, and community partners, ensuring continuity of care for your clients.

• Participate actively in the agency’s licensing and accreditation processes, contributing to our goal of maintaining high standards of care.

• Promote the agency’s mission and values within the community through outreach and engagement with local organizations.

• Commit to ongoing personal and professional development, attending relevant conferences and trainings to stay current in the field.

• Carry out other responsibilities as assigned by the Clinic Director, demonstrating flexibility and dedication to team operations.

Patient Population / Client Focus\n

The primary focus of this role is to provide specialized counseling services to seniors dealing with issues such as grief, adjustment to changes in health, and other age-related challenges. Qualifications\n

• A Master's degree in Mental Health Counseling from a recognized and accredited institution.

• A valid Licensed Mental Health Counselor (LMHC) credential.

Compensation and Benefits\n
