Key Responsibilities\n

• Conduct comprehensive assessments to determine the unique mental health needs of clients, ensuring tailored treatment plans are developed and implemented.

• Maintain clear communication with supervisors, peers, and family members to provide updates on client progress and to collaboratively address any modifications needed in treatment approaches.

• Provide supervisors with a detailed schedule of client appointments, home visits (where applicable), and follow-ups, ensuring efficient management of time and resources.
\n
• Coordinate and carry out virtual or in-home visits for thorough assessments, client engagement, and ongoing support when necessary.
\n
• Monitor the implementation of individualized program plans and adjust strategies as needed based on client feedback and treatment efficacy.
\n
• Participate actively in interdisciplinary team meetings to collaborate on best practices and share insights that enhance client care.
\n
• Document all client interactions comprehensively in electronic medical record (EMR) systems, including monthly and quarterly progress notes to track improvements and inform future sessions.

• Train and mentor support staff to ensure they are well-equipped with the necessary skills to adhere to the program plans and maintain high standards of documentation.

• Provide follow-up support through interviews, virtual home visits, and continuous observations of clients to adapt treatment as their needs evolve.

• Serve as a liaison with the mental health community to advocate for clients and assist in achieving their developmental goals.

• Contribute to the development of training materials and procedural guidelines aimed at continuous improvement of programs offered to clients.

Patient Population / Client Focus\n

In this role, you will work with a diverse clientele, including adults and youth, addressing a wide range of mental health issues. Common client challenges may encompass anxiety, depression, trauma, substance abuse, and relationship dynamics. Your expertise, especially in family therapy or trauma-focused approaches, will be particularly beneficial in tailoring therapeutic interventions that truly resonate with and empower clients to take control of their mental health journeys.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Clinical Professional Counselor in the state of practice.
