You will engage with clients through telehealth platforms, utilizing advanced electronic medical record (EMR) technologies to record patient progress, chart notes, and facilitate ongoing care.

Key Responsibilities\n

• Conduct thorough and ongoing assessments of patients to identify their unique mental health needs, facilitating tailored treatment plans.

• Develop and maintain effective communication streams with supervisors, family members, and other stakeholders regarding patient progress, treatment adjustments, and goals.

• Provide a structured tentative schedule of appointments and home visits to your supervisors, ensuring the effective tracking of your interventions.

• Coordinate essential home visits, incorporating chart reviews and staff interviews to gather holistic insights into patient well-being.

• Ensure the seamless implementation of individual program plans, actively monitoring and adjusting them as necessary to optimize effectiveness.

• Participate in interdisciplinary team meetings, offering insights and recommendations based on clinical observations and patient interactions.

• Document progress through timely monthly and quarterly progress notes, ensuring that treatment records are complete and accessible.

• Organize training sessions for staff members and team colleagues, enhancing competency in program plan implementation and documentation practices.
