Key Responsibilities\n

• Conduct thorough assessments to identify client needs and develop tailored treatment plans based on best practices and client feedback.
\n
• Maintain proactive communication with family members, guardians, and other stakeholders regarding client progress to ensure comprehensive support.
\n
• Provide a detailed schedule to supervisors outlining appointments, home visits, and observations necessary for monitoring client progress and service delivery.
\n
• Coordinate necessary home visits to conduct in-home observations, chart reviews, and staff interviews that contribute to individual case planning and analysis.
\n
• Monitor the implementation of treatment plans, ensuring they are effectively executed and adjusted as needed based on client feedback.
\n
• Actively participate in Interdisciplinary Team Meetings to contribute to a holistic approach in client care.
\n
• Create and maintain thorough documentation including monthly and quarterly progress notes to track client advancements and treatment efficacy.
\n
• Organize training sessions for staff to enhance competencies related to the implementation of treatment plans and documentation processes.
\n
• Conduct follow-up interviews and home visits to ensure continuity of care and address any ongoing client needs effectively.

• Serve as a liaison with the broader mental health community to support development goals and foster collaborative relationships.

Patient Population / Client Focus\n

You will work with a diverse client base, including both adults and youth, presenting a wide array of mental health concerns. Our clientele may face challenges such as anxiety, depression, trauma, and substance misuse. As an LCPC, you will be instrumental in empowering them to navigate their complexities through empathetic counseling and evidence-based interventions.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field from an accredited institution is required.

• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in Illinois is essential.

• A minimum of two years of clinical counseling experience is required, showcasing a history of effective client engagement and support.

• Specialization or significant experience in family therapy, couples therapy, trauma-informed care, or substance abuse treatment is highly preferred.

• Strong understanding of applicable legal and ethical standards in mental health counseling practices.
