Key Responsibilities\n

• Conduct thorough assessments of clients’ counseling and mental health needs to create informed treatment plans.

• Utilize DSM criteria to assess clients for substance use diagnoses, ensuring appropriate levels of care are determined based on ASAM criteria.

• Design and prepare the substance abuse counseling segment of treatment plans tailored to individual client needs.
\n
• Develop and implement discharge and continuum of care plans, facilitating client transition to suitable levels of care.
\n
• Document all counseling sessions and related services, maintaining accurate clinical records that reflect each client's journey.
\n
• Manage a caseload of clients, performing case management duties, treatment planning, and clinical charting as necessary.
\n
• Coordinate support services, including liaising with family members, legal representatives, schools, and employers.
\n
• Facilitate multi-disciplinary approach interventions for crisis situations, ensuring timely and effective client support.
