Work Environment

Your 36 to 40 hour work schedule will typically consist of rotating weekday and weekend shifts, allowing for consistency in patient relationships while promoting a healthy work-life balance.

Key Responsibilities

• Conduct thorough assessments of patients in their homes, accurately diagnosing a range of acute and chronic conditions.
• Develop individualized, evidence-based treatment plans in collaboration with attending physicians and caregivers, ensuring continuity of care.
• Order, interpret, and follow up on laboratory and imaging studies, integrating results into treatment strategies.
• Prescribe medications and therapies, closely monitoring patient responses and adjusting plans as necessary to optimize care outcomes.
• Provide comprehensive patient and family education that fosters understanding of disease management, medication compliance, and post-discharge planning.
• Engage in interdisciplinary case reviews and collaborate effectively with team members to enhance patient care processes.
• Maintain precise and up-to-date electronic medical records (EMR), ensuring compliance with regulations and standards.

Patient Population / Client Focus

Qualifications

• Current certification as a Nurse Practitioner, credentialed through an established board.
• Active state nursing license in New York.

Compensation and Benefits

Gotham Enterprises Ltd offers a competitive salary range of USD 120,000 to 220,000 per year, commensurate with your experience and qualifications.
