Key Responsibilities\n

• Conduct ongoing assessments of clients to develop effective treatment plans tailored to their needs, utilizing evidence-based practices.

• Document and share tentative schedules of appointments, including home visits and telehealth sessions, to ensure comprehensive support.

• Coordinate home visits, ensuring thorough observations, chart reviews, and staff interviews to collect data for treatment adjustments.

• Actively participate in Interdisciplinary Team Meetings, contributing to a collaborative approach for client care.

• Prepare and maintain monthly and quarterly progress notes to track client development and support effective treatment.

• Lead training efforts, ensuring that all team members are knowledgeable in the implementation of the program and best practices in client documentation.

• Provide follow-up support through interviews and home visits, ensuring continuity of care for clients.

• Organize and maintain records meticulously to provide accurate and accessible information for clients and other interested parties.

• Serve as a liaison with public and mental health communities, advocating for clients and establishing development goals.

• Assist in developing training materials and procedures aimed at improving individual therapy programs.
