Key Responsibilities\n

• Conduct thorough ongoing assessments of clients to identify their mental health needs and develop individualized treatment plans.

• Communicate effectively with a range of stakeholders, including supervisors, family members, and other involved parties, regarding patient progress and treatment goals.

• Maintain a structured schedule of appointments and home visits, ensuring adequate planning for monitoring client advancements.
\n
• Coordinate and execute home visits as required for observations and comprehensive assessments, focusing on the holistic well-being of clients.

• Ensure that therapeutic interventions are effectively implemented by regularly reviewing the program plan and making necessary adjustments.

• Engage in interdisciplinary team meetings to collaborate on treatment strategies and share insights about client progress.
\n
• Prepare and submit monthly and quarterly progress notes to track client outcomes and inform ongoing care strategies.
\n
• Organize training sessions for staff to enhance competencies in program implementation and documentation standards.
\n
• Provide continual support through follow-up interviews and additional home visits to address any emerging client needs.

• Act as a liaison between clients and the broader mental health community, facilitating resources and supports for individual development.

• Contribute to creating training materials and procedures that enhance service delivery and client care.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, Social Work, or a related field.

• Active and unrestricted state licensure as a Professional Clinical Counselor (LPCC).

• A minimum of two years of clinical experience in a counseling capacity is required.

• A strong understanding of ethical and legal standards relevant to mental health practices.

• Proficiency in electronic medical records (EMR) software and related platforms to ensure seamless documentation and client management.

Compensation and Benefits\n

• The salary range for this position is between $90,000 and $110,000 annually, commensurate with experience and caseload.
