Job Description

We are seeking a skilled Nurse Practitioner to perform home health risk assessments for Medicare Advantage plan members across Cass County, Indiana. This per diem opportunity allows for maximum flexibility, enabling you to design your work schedule and choose shifts between Monday and Sunday. Youll be instrumental in supporting the health and wellness of patients residing in Galveston, Logansport, Walton, and surrounding areas through comprehensive home-based evaluations.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling choose your own hours
• Minimum weekly or bi-weekly time commitment

Key Responsibilities:

• Complete annual risk assessments within patients homes, most lasting around 45 minutes
• Evaluate comprehensive medical histories, current health status, and medication use
• Educate patients on both conditions and medication management
• Identify care gaps and make actionable recommendations for improved outcomes
• No requirement for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Indiana
• Current CPR (BLS) certification
• Board-certified through AANP or ANCC
• MSN degree from an accredited nursing program
• At least 6 months of current Nurse Practitioner experience preferred
