Key Responsibilities\n

• Conduct comprehensive assessments of clients’ social, emotional, and environmental challenges to inform service provision.

• Develop and implement personalized service plans that include specific goals, interventions, and progress evaluations.

• Provide quality counseling and emotional support to individuals and families experiencing various life challenges, facilitating therapeutic interventions as needed.

• Collaborate with clients to empower them by connecting them to vital community resources, health services, and educational opportunities.

• Perform crisis intervention and risk assessments, creating safety plans to ensure the well-being of clients facing immediate danger.

• Engage actively with community agencies and organizations, advocating for clients to ensure they receive comprehensive services.

• Participate in community outreach efforts to raise awareness about agency services and address social issues in our community.

• Attend case conferences, engage in team meetings, and participate in ongoing training to continuously enhance professional skills.

Patient Population / Client Focus\n

The LCSW will primarily serve clients who are part of underserved communities in Chicago, focusing on those struggling with complex social issues. This includes individuals and families experiencing mental health challenges, homelessness, substance abuse, and domestic violence. By utilizing a trauma-informed approach, the social worker will empower clients to achieve stability and enhance their well-being.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

Compensation and Benefits\n

The salary range for this position is competitive, ranging from USD 60,000 to 100,000 annually, depending on experience and qualifications.
