Key Responsibilities\n

• Assist primary care providers in recognizing and managing mental health issues within the broader context of a patient's overall health, ensuring a collaborative approach to treatment.

• Conduct thorough patient intakes, focusing on detailed diagnostic and functional evaluations to guide treatment planning.

• Provide brief, focused therapeutic interventions tailored to the immediate needs of clients requiring mental health support.

• Advise primary care providers on patient referrals to specialty mental health services when necessary, ensuring continuity of care.

• Initiate follow-up sessions to monitor patient progress and recommend any adjustments to treatment strategies.

• Develop individualized relapse prevention plans for patients identified as at risk for crisis or relapse, equipping them with strategies to maintain stability.

• Coordinate health services across the behavioral health spectrum, linking patients to additional resources with their consent.

• Maintain comprehensive documentation in treatment charts to track patient progress, providing timely updates to primary care teams.

• Work collaboratively with other team members to provide behavioral services and participate in program evaluation discussions.

• Implement educational initiatives that empower patients with self-management skills and engage them actively in their recovery.

Patient Population / Client Focus\n

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling or a closely related field.

• Current and valid LPC (Licensed Professional Counselor) License in the state of Texas.

• Ability to effectively conduct psychodiagnostic assessments and utilize behavioral therapy approaches.

Compensation and Benefits\n

The annual salary for this role ranges from USD 87,000 to USD 105,000, reflecting your experience and qualifications.
