Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for individuals, adolescents, families, and groups, utilizing graduate-level clinical techniques to identify and address client needs effectively.

• Facilitate initial assessments and triage care based on patient complexities and requirements, ensuring timely and appropriate interventions.

• Engage in active communication with interdisciplinary team members regarding patient status, treatment plans, and necessary adjustments, documented clearly in electronic patient records.

• Demonstrate competence in meeting the physical, psychological, social, educational, and safety needs of a diverse patient demographic while adhering to ethical standards.

• Collaborate with community resources and professionals to provide comprehensive case management services for assigned patients, addressing barriers to care and promoting optimal outcomes.

• Complete necessary documentation promptly and accurately, maintaining compliance with departmental requirements and regulatory standards.

• Implement Cognitive Behavioral Therapy (CBT) and other evidence-based modalities to support patients and families in achieving their individualized service plans.
