Work Environment

This position is set in an Assisted Living Facility, providing a supportive and collaborative atmosphere where you will work closely with a multidisciplinary team of healthcare professionals. Your schedule is primarily Monday to Friday from 9:00 a.m. to 6:00 p.m., with the possibility of evening clinics until 8:00 p.m. Occasional weekend outreach events or health fairs may also be part of your responsibilities. With an average workweek totaling approximately 40 hours, we emphasize work-life balance while fostering a dynamic healthcare environment.

Key Responsibilities

• Perform thorough health assessments and screenings for patients, identifying individual and family health concerns.
• Diagnose, manage, and develop treatment plans for diverse acute and chronic medical conditions, including preventive care strategies.
• Champion health education programs by collaborating with community organizations, ensuring participants understand their health and wellness options.
• Deliver culturally sensitive and patient-centered care, advocating for equitable access to healthcare services.
• Coordinate referrals and ongoing follow-up care to maintain a seamless healthcare experience for patients.
• Document patient interactions and treatment plans meticulously in compliance with healthcare regulations.
• Engage with the community by participating in outreach initiatives, such as health fairs, to promote wellness and disease prevention.
• Stay informed on latest community health trends and evidence-based practices to continuously improve service delivery.

Patient Population / Client Focus

Your focus will be on serving a diverse patient population that includes seniors residing in the Assisted Living Facility and their families. You will address a range of health concerns, from chronic disease management to preventive care and health promotion, ensuring that each patient receives personalized and comprehensive support. Qualifications

• Current certification as a Family Nurse Practitioner is required.
• Active nursing license in the state of New York.
• A minimum of a Master’s degree in nursing from an accredited program.
• At least 3 years of clinical experience, ideally in community health or public health settings.
• Exceptional interpersonal and communication skills, with a proven ability to connect with diverse populations.

Compensation and Benefits
