Work Environment

This position is based in Queensbury, New York, where scenic natural beauty meets vibrant community life. As a Nurse Practitioner in our home care department, you will work in a dynamic and supportive environment where collaboration among healthcare providers is emphasized. Your schedule will typically run from 9:00 a.m. to 6:00 p.m., with flexibility necessary for occasional evening clinics lasting until 8:00 p.m. Additionally, participation in community outreach events during weekends may be required. The average workweek will total approximately 40 hours.

Key Responsibilities

• Conduct thorough health assessments and screenings, taking a comprehensive approach to evaluate both physical and mental health needs of patients.
• Diagnose and manage both acute and chronic medical conditions while also emphasizing preventive care and health promotion strategies for patients.
• Collaborate closely with community organizations to design and implement educational programs, fostering a culture of wellness and preventive healthcare practices.
• Deliver culturally sensitive care that takes into account the diverse backgrounds of patients, ensuring that they are informed advocates for their health needs.
• Coordinate referrals and follow-up care seamlessly to ensure patients experience continuity of care, significantly improving their health outcomes.
• Document all patient interactions and maintain accurate medical records in compliance with regulatory standards, providing essential data for quality assurance initiatives.
• Participate in community outreach events and health fairs, providing valuable health screenings and information to promote awareness of health-related issues.
• Stay informed about evolving community health trends, evidence-based practices, and relevant state and federal health policies to provide the best care to patients.

Qualifications

• A current certification as a Nurse Practitioner and an active state nursing license.
• A Master’s degree in nursing from an accredited program.
• A minimum of 3 years of clinical experience is required, preferably in community health or public health settings.
• Excellence in communication and interpersonal skills, with a demonstrated ability to work effectively with individuals from diverse cultures and backgrounds.
• A strong commitment to social justice, health equity, and advocacy for marginalized communities.

Preferred Experience

While the minimum requirement is three years of experience, candidates possessing 5 or more years in a similar role are especially encouraged to apply. Compensation and Benefits

This position offers a competitive salary range of $120,000 to $130,000 per year, dependent on your experience and qualifications.
