Work Environment

This role is primarily onsite, offering a dynamic and engaging work environment. As you perform your duties in the field, you will work closely with a team of medical professionals while having access to the latest technology and medical resources to enhance patient care. You will collaborate with physicians, nurses, and other support staff to ensure comprehensive service delivery that meets the specific needs of each patient.

Key Responsibilities

• Conduct thorough physical examinations, assess patient health, and interview patients to gather comprehensive medical histories.
• Develop and execute effective patient management plans, ensuring continuity of care for individuals with both acute and chronic conditions.
• Develop treatment protocols in collaboration with supervising physicians, focusing on both immediate and long-term patient health goals.
• Order and interpret diagnostic tests, take responsibility for follow-up visits, and provide timely updates to healthcare providers regarding changes in patient status.
• Educate patients and their families about health conditions, treatment options, and preventive care practices.
• Document patient encounters accurately and in a timely manner in the electronic health record (EHR) system.
• Participate in team meetings to review patient progress and discuss case management strategies.

Patient Population / Client Focus

You will primarily serve a diverse patient population, focusing on older adults and individuals with multiple chronic conditions. Many of our patients require ongoing management of their health, including diabetes, hypertension, and heart disease. Our goal is to provide comprehensive medical care that integrates physical health with overall wellness, assisting patients in navigating their healthcare journey.

Qualifications

• Bachelor's Degree in Nursing or Physician Assistant Studies from an accredited institution.
• Current certification as a Nurse Practitioner or Physician Assistant.
• Active state licensure to practice in New York.
• Proficient in using EHR systems for documenting patient care.

Preferred Experience

• Minimum of two years of experience working in an ambulatory care setting, particularly in internal medicine or family practice.
• Experience managing a diverse patient caseload, particularly with chronic illnesses.
• Certification in Advanced Cardiac Life Support (ACLS) is an advantage.

Compensation and Benefits
