Work Environment

This position is set in our community-oriented outpatient office located in Queensbury, NY, providing an environment that fosters collaboration and teamwork. The typical work hours will span from 9:00 AM to 6:00 PM, with occasional evening clinics extending until 8:00 PM, allowing for flexibility in scheduling. Participation in weekend community outreach events or health fairs is also a possibility, further strengthening our ties to the community we serve.

Key Responsibilities

• Perform comprehensive health assessments and screenings for patients of all ages, focusing on both acute and chronic conditions.
• Diagnose and manage a variety of medical issues, establishing treatment plans that include preventive care and ongoing health promotion.
• Engage in collaborative efforts with community organizations to develop health education programs aimed at raising awareness and improving public health.
• Provide culturally competent care, advocating for patient rights and facilitating access to necessary healthcare services.
• Coordinate referrals and ensure appropriate follow-up care to guarantee continuity in patient treatment.
• Maintain meticulous and compliant medical documentation, ensuring adherence to all regulatory standards.
• Participate actively in outreach programs, including health fairs that provide free screenings and health education to underserved populations.
• Stay informed about current community health trends, evidence-based practices, and relevant policies to enhance the quality of care provided.

Patient Population / Client Focus

The FNP will serve a diverse client base within the community, providing healthcare services that cater to the unique cultural and socioeconomic needs of different populations. Qualifications

• Active certification as a Family Nurse Practitioner.
• Current state nursing license in New York.

Compensation and Benefits

We offer a competitive annual salary range of USD 180,000 to 200,000, which is commensurate with your experience and qualifications. In addition, we provide a comprehensive benefits package that includes:
