Work Environment

Our outpatient office is designed to foster a healing atmosphere for patients and staff alike. You will frequently interact with a diverse patient population, requiring sensitivity to cultural differences and a strong ability to communicate effectively. The work schedule typically runs from 9:00 a.m. to 6:00 p.m., with occasional evening clinics available until 8:00 p.m. Additionally, you may be asked to participate in community health events during weekends, reinforcing our commitment to accessible care.

Key Responsibilities

• Conduct thorough health assessments and screenings, able to identify both acute and chronic medical conditions based on thorough history-taking and physical examination techniques.
• Formulate and implement individualized care plans that address physical, emotional, and social health needs, promoting preventive care and health education measures.
• Work collaboratively with local organizations to design and execute health education initiatives aimed at increasing community awareness about prevalent health issues.
• Provide culturally competent care to patients, ensuring that healthcare services are accessible and equitable.
• Coordinate referrals and facilitate follow-up care, collaborating with other healthcare professionals as necessary to ensure continuity in patient care.
• Maintain precise medical records consistent with HIPAA regulations and organizational policies, ensuring that documentation is both accurate and insightful for ongoing patient care.
• Engage actively in outreach programs and health fairs, contributing to the organization’s goal of raising community health awareness.
• Stay informed on current healthcare trends, public health policies, and evidence-based practices to enhance the quality of care provided.

Qualifications

• Must possess a current certification as a Nurse Practitioner and a valid state nursing license for New York.
• A Master's degree in nursing from an accredited institution is required to meet educational standards.
• A minimum of three years of clinical experience is mandatory, particularly in community or public health environments, with strong evidence of applied practice.
• Exceptional interpersonal and communication skills are essential, enabling you to work effectively with patients and collaborate with multidisciplinary teams.

Preferred Experience

Compensation and Benefits

We offer a competitive salary range of $180,000 – $200,000 annually, commensurate with your professional experience and qualifications.
