Work Environment

This Full-time position is based onsite at our assisted living facility in Queensbury, New York, where you will work closely with a diverse population of residents, their families, and caregivers. Your typical work hours will be from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending to 8:00 p.m., and participation in weekend community outreach events may be required. This role offers an average workweek of approximately 40 hours, providing a stable schedule while allowing for flexible community engagement.

Key Responsibilities

• Conduct thorough health assessments and screenings for residents, ensuring comprehensive understanding and addressing their acute and chronic health issues.
• Diagnose and manage a wide range of medical conditions, providing preventive care and promoting healthy lifestyle choices among the patient population.
• Work collaboratively with local community organizations to develop and implement health education initiatives that empower individuals with knowledge about their health.
• Deliver culturally sensitive care that respects the values and needs of our diverse patients, advocating for their rights and access to necessary healthcare services.
• Coordinate patient referrals and ensure meticulous follow-up care to maintain continuity in treatment plans and achieve optimal health outcomes.

Patient Population / Client Focus

Your primary focus will be on residents of the assisted living facility, many of whom are elderly or have chronic health conditions requiring ongoing management. Qualifications

• Current certification as a Nurse Practitioner, demonstrating your commitment to professional standards in healthcare.
• Active New York state nursing license, allowing you to practice effectively and responsibly within our facility.
• A minimum of a Master's degree in nursing from an accredited program, providing you with the foundational knowledge required to succeed in this role.

Preferred Experience

Compensation and Benefits

This role offers a competitive salary range of $120,000 to $130,000 per year, commensurate with experience and qualifications.
