Key Responsibilities\n

• Conduct thorough assessments of patients’ mental health needs to tailor individualized treatment plans.

• Provide telehealth and, when necessary, in-person behavioral health services to clients and families facing terminal illnesses.

• Document all services rendered thoroughly within our Electronic Health Record (EHR) system to maintain up-to-date client profiles.
\n
• Foster therapeutic relationships by delivering services with cultural competence, understanding diverse backgrounds, identities, and values.
\n
• Regularly write detailed progress notes, treatment summaries, and case reports to track patient progress.
\n
• Engage in crisis intervention when needed, providing necessary support and connecting clients to additional resources.
\n
• Stay informed on the latest trends and best practices in mental health to continuously enhance service delivery.

Patient Population / Client Focus\n

In this role, you will predominantly work with patients receiving hospice care, as well as their families, who often experience a wide range of emotions, including anxiety, grief, and depression. Your services will be crucial in helping them process these feelings and lead them toward a place of peace and acceptance during a pivotal time in their lives.

Qualifications\n

• Master’s degree in Social Work from an accredited institution.

• Active and unrestricted LCSW license to practice in the state of California.

• Excellent clinical skills with a strong emphasis on telehealth service delivery.

• Proficiency in staying organized, maintaining an updated calendar, and managing client session schedules efficiently.

• Ability to complete progress notes and documentation promptly after each session to ensure clear communication in the patient care continuum.

Preferred Experience\n

• A minimum of two years' experience providing social work services, particularly within hospice or palliative care settings.

• Proven experience in crisis intervention and support for families experiencing loss.

Compensation and Benefits\n

The compensation for this position ranges from $110,000 to $130,000 annually, in addition to a comprehensive benefits package that includes:
