Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify the needs and strengths of patients and their families.

• Develop and implement individualized treatment plans that incorporate evidence-based therapeutic approaches, including individual, group, and family therapy.

• Coordinate and oversee case management services for assigned patients, ensuring they receive the appropriate support and resources.

• Research, refer, and advocate for community resources, including healthcare, housing, and financial assistance programs, to enhance patients' well-being.

• Respond to crisis situations effectively by evaluating emergencies such as mental health crises or cases of child abuse, providing immediate and supportive interventions.

• Maintain ongoing communication with patients to monitor their progress, adjust treatment plans as necessary, and provide encouragement through challenges.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution is required.

Compensation and Benefits\n

This rewarding position offers a competitive salary range of $120,000 to $130,000 annually, reflecting your experience and expertise in the field. Additional benefits include:
