Key Responsibilities\n

• Conduct thorough psychosocial assessments of patients and their families upon admission and on a regular basis, identifying social, emotional, and physical needs while applying relevant social work theories and techniques.

• Prepare and maintain electronic medical records, ensuring compliance with departmental policies and health system standards while providing comprehensive reports and effective hand-offs.

• Work collaboratively with interdisciplinary healthcare teams to devise, implement, and monitor patient and family psychosocial treatment plans, coordinating referrals to specialty services and documenting progress clearly.

• Provide supportive counseling, grief counseling, and crisis intervention tailored to assist patients and families in addressing environmental and psychological challenges related to illness.
\n
• Manage a diverse caseload of patients with varying levels of clinical acuity, ensuring continuity of care by regularly reviewing and updating treatment plans.
\n
• Participate actively in team conferences and interdisciplinary rounds, engaging in case finding for patients who may have psychosocial needs.
\n
• Deliver psychoeducation to help patients and families navigate the complexities associated with diagnoses, treatments, and prognoses while facilitating transitions from childhood to adult mental health services.

• Involve yourself in quality assurance initiatives and community improvement activities related to social work services, contributing to the development and review of relevant policies and procedures.

• Make mental health diagnoses as part of treatment planning as needed, using established protocols and evidence-based practices.

• Provide individual and group clinical supervision to Master’s level Social Work students (MSWs) and interns, fostering their professional development.

Qualifications\n

• A Master’s degree in social work or a closely related behavioral science field from an accredited institution is required.

• Possession of one or more of the following licenses: LCSW (Licensed Clinical Social Worker), LMFT (Licensed Marriage and Family Therapist), or LPCC (Licensed Professional Clinical Counselor).

• Exceptional written and oral communication skills, including the ability to prepare detailed case narratives and progress reports.
