Work Environment

This full-time position requires onsite work in patients' homes throughout Arlington, Virginia. Key Responsibilities

• Conduct thorough evaluations of patients' physical and functional abilities by reviewing pertinent medical histories, performing specific assessments, and documenting your findings to create tailored treatment plans.
• Communicate evaluation results clearly to patients and their families, actively involving them in the goal-setting process to ensure individualized and attainable recovery objectives.
• Implement evidence-based treatment protocols according to evaluation outcomes and adapt as needed in response to changes in patients' conditions.
• Work proactively with other healthcare professionals to consult on complex cases and modify care plans as necessary, ensuring that all changes are documented accurately.
• Execute comprehensive discharge planning, providing necessary follow-up care, and maintaining precise documentation of patient encounters to paint a clear picture of their progress throughout the treatment journey.
• Demonstrate exceptional time management skills by maintaining an organized schedule and meeting productivity expectations.
• Participate actively in community outreach and hospital marketing initiatives to enhance awareness and accessibility of our services.
• Engage in personal and professional development activities, including attendance at workshops, seminars, and offering mentorship to physical therapy students.
• Foster a patient-centered approach that prioritizes exceptional customer experiences, contributing to the overall satisfaction of those we serve.

Patient Population / Client Focus

In this role, you will primarily interact with diverse patient populations in their homes, ranging from post-operative patients to individuals recovering from chronic conditions. Your ability to create individualized therapy plans that address the specific needs of each patient will be crucial in promoting their rehabilitation and achieving meaningful recovery outcomes.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
• Current state licensure as required for providing physical therapy services in Virginia.
• American Heart Association's Basic Life Support (BLS) for Healthcare Providers certification is required.

Preferred Experience

• 2 years of experience in physical therapy, preferably in a home care setting, although new graduates are welcome to apply.
• Experience working collaboratively in a multi-disciplinary team environment is advantageous.

Compensation and Benefits
