Implement care plans and administer medications and treatments per physician orders and facility policies. Monitor resident status, identify changes, and report to the care team promptly. Document care accurately in electronic medical records and communicate changes across disciplines. Collaborate with physicians, licensed professionals, and support staff to achieve optimal resident outcomes. Provide age‑appropriate, culturally sensitive care and participate in quality improvement activities to support patient safety and outcomes.
