Key Responsibilities\n

• Collaborate effectively with physicians and interdisciplinary team members to discuss client progress, address ongoing concerns, and resolve issues impacting patient care.

• Maintain comprehensive and accurate client records, including clinical assessments and progress notes, in alignment with organizational policies and HIPAA regulations.

• Develop and implement individualized treatment plans that include psychosocial assessments, evidence-based interventions, and discharge planning strategies, ensuring alignment with established best practices in mental health care.

• Provide compassionate counseling and crisis intervention services, with a focus on assessing and treating emotional and behavioral disorders.

• Facilitate care coordination and assist clients with discharge planning, collaborating closely with regional service providers to ensure a smooth transition to community services.

• Engage with families and support systems to discuss treatment options, share progress, and strategize effective discharge planning that encompasses the clients’ needs.

As a licensed professional counselor, you will primarily work with a diverse range of clients grappling with various mental health challenges, including anxiety, depression, trauma, and behavioral disorders.
